- What specific need for the betterment of individuals in your community did you identify as a leader? Describe the efforts you have participated in to address and resolve this need. When you envision the future of your community because of your efforts, what positive changes do you foresee?
- Color headshot suitable for website posting
- Educator recommendation letter
- Academic resume
- Current Transcript (can be official or unofficial)
